How To Get A Passport In Utah
The Department of State has resumed full passport operations with longer than normal processing times. As of 9/23/2021 the standard processing times are as follows:
* Routine: Door-to-door service is up to 16 weeks from the day an application is submitted to the day a new passport is received.
* Expedited: Costs an extra $60. Door-to-door service is up to 12 weeks from the day an application is submitted to the day a new passport is received.
These door-to-door timeframes include both our processing times and mailing times on the front and back end. The Department of State recommends passport applicants apply AT LEAST 6 MONTHS BEFORE they plan to travel. Click on the "Make and Appointment" button below to schedule an appointment. Appointments are recommended, but not required. Individuals with appointments are given preference over walk-in applicants.
General Passport Information - Read This First!
- REQUIRED!! A check, cashier's check or money order for application fee (See Passport Fees section) payable to US Dept of State.
- An adult passport (16 years of age and older) is valid for 10 years.
- A child's passport (ages 0-15 years old) is valid for 5 years.
- DO NOT sign the passport application until requested to do so.
- If a name is different than the name on the birth certificate, you may be required to provide a legal document (such as a certified copy of a marriage license) to verify the name change.
Finding the Right Form
Applying for a U.S. Passport can take a lot of time at the Passport Office. Save yourself some time! Find the form or forms you need online first. Fill them out, print them out, and bring them with you to the Passport Office.
What Form Should I use?
Processing Times:
Refer to the U.S. Department of State's website for the most up to date processing times. The best advice is, "APPLY EARLY!"
Proof of Citizenship:
- If born within the United States you must provide a certified copy of your birth certificate. (DOCUMENTS NOT ACCEPTABLE include: hospital certificate, baptism, notification-of-birth and California abstracts of birth.)
- If born outside of the United States you must provide a Certificate of Citizenship/Naturalization or report of birth abroad.
- For a complete list of all forms of acceptable citizenship documentation visit the U.S. Dept of State's website.
- ALL APPLICANTS (including children) must personally appear when applying for a passport.
- Children under 16 years of age must have both parents present.
- IF one of the child's parents can not personally appear, download and complete Form DS- 3053 Statement of Consent for Issuing a Passport to a Child.
- IF one parent can not be located or contacted to personally appear, download and complete Form DS-5525 Statement of Special Family Circumstances.
- Children 16 and 17 years of age must have one parent present.
- Social security numbers must be provided for all passport applicants!
If you have had a passport within the last 15 years and were at least 16 years of age or older at the time of issuance, you may obtain a form and submit your own application.
For this purpose you may obtain a passport application form from the Utah County Clerks Office, or you can obtain a copy or visit the U.S. Department of State's website.

Passport Photo Requirements
- One U.S. passport photograph is required with the application.
- Passport photographs must have been taken in the last 6 months.
- Pictures must be in color and on a white or light colored background.
- Pictures must be two-inches square. (Image size of the head must be between 1 inch and 1 3/8 inches including hair.) See a qualified passport photographer.
Identification Requirements
- In-state, fully valid driver's license or enhanced driver's license with photo (not temporary or learner's permit)
- Official U.S. military or military dependent identification card
- Government employee identification card (federal or state)
- Current foreign passport
- FAA pilot's license

Passport Fees
A U.S. Passport Book is the standard type of Passport. If you intend to travel by air to an international destination, the passport book is the only document approved for international air travel. The fees for a US Passport Book are as follows:
16 years or older - Two separate payments required
- $110.00 (payable to U.S. Department of State, check or money order only)
- $35.00 (payable to Utah County Clerk's Office - cash, check, money order, MasterCard, or Visa)
- $80.00 (payable to U.S. Department of State, check or money order only)
- $35.00 (payable to Utah County Clerk's Office - cash, check, money order, MasterCard, or Visa)
Passport cards will facilitate entry and expedite document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean, and Bermuda. The passport card may not be used to travel by air.

A Passport Book by itself is a fully usable US Passport for all situations. However, you can order and use both a Passport book and a Passport Card in the appropriate situation if you choose.

An expedited application typically speeds up the processing and issuance of your Passport. The expedite fee adds an additional $60.00 per expedited application.
